PET TRANSPORT SERVICES In order for LNT Pet Transport Services to proceed with the scheduling of transporting your pet, we ask that you READ AND AGREE TO THE FOLLOWING terms and conditions as outlined below. We require a printed signed copy of this agreement be mailed to us at 13624 Hwy 87E, Adkins, TX 78101 , prior to your scheduled pick-up week. If time does not permit mailing due to scheduling, we ask that you have it available upon pick-up of your pet. 1) NO DOG WHICH IS LESS THEN 8 WEEKS OF AGE CAN BE TRANSPORTED. By signing this form, the individual requesting transport certifies that the pet being transported by LNT Pet Transport Services is at least 8 weeks of age on or before the timeframe it has been scheduled for pick-up. 2) A printed copy of our emergency care authorization form must be completed and must travel with your pet. If you are sending multiple pets, they may all be listed on a single form. This form gives us permission to take your animal to a qualified veterinarian in the event they become ill during their transport. Should this be necessary, we will make every effort to contact you prior to taking any emergency action. However, if you can not be reached to have the medical emergency/treatment approved by you, we will use our descretion to ensure the well being of your pet. 3) A Valid HEALTH CERTIFICATE issued by a Dr. of Veterinary Medicine no sooner than 10 days before the scheduled delivery date must be obtained and travel with your pet. Complete medical records or shot records are not acceptable and CAN NOT be used as a substitution. 4) We require a copy of the current rabies certificate to accompany your pet. PLEASE have the Emergency Care Authorization, Health Certificate and copy of Rabies Certificate in a manilla folder or envelope upon pick-up of your pet so that nothing will be left behind. 5) We require a 5 lb bag of food be sent with each pet as well as a properly fitted collar. We will provide everything else to ensure your pet has a comfortable trip. Please label your food bag with your pet's name and feeding instructions, ie. amounts and frequency of meals. Please note that, unless otherwise specified, your pet will have access to their water and food 24 hours a day. Special dietary requirements must be noted on a seperate sheet and given to our drivers when they arrive for pick-up. We will provide bottle water, ice and treats unless otherwise specified. 6) I have reviewed the payment terms as outlined on the Rates/Payment page. I understand that full payment is due prior to the scheduled pick up unless arrangements are made and agreed upon in writing during the scheduling process. I understand that should there be a balance due for transport and it has not been received by LNT Pet Transport prior to scheduled pick up week, LNT will assume that transport has been cancelled and will not be expected to pick up of my pet. I also understand that quoted prices are based on description of breed during scheduling and should the transporter feel the size crate reserved is not adequate, we will upgrade to a larger crate at the time of pick-up. Payment for upgrades will be collected upon pick up or drop off. 7) I understand that there will be an additional charge for the administration of any medications at a rate of $5.00 per day per pet. Any medications we are asked to administer must be dispensed specifically for your pet by a licensed Veterinarian and must be sent in the container it was dispensed in reflecting your pets name, dosage and frequency. 8) COD payments: Although it is not customary to accept COD payments, for whatever reason should this be necessary, I understand that if monies are not available upon delivery, I will be responsible for any and all charges incurred by LNT Pet Transport to keep and care for my pet until such time that other arrangements for delivery can be made. Should the pet require transport back to the original pick up contact, I understand that it is my sole responsibility to cover such transport fees. The return transport price will not exceed our normal transport charge as outlined on our rates page however may be subject to additional charges. Furthermore, should LNT Pet Transport re-schedule a delivery there will be a charge of $100.00 per day for each pet to defray the cost of its care and the animal will be rescheduled on our next available transport. All money due will be required prior to drop off. 9) Scheduling: 10) Duration of Transport: 11) LNT PET TRANSPORT SERVICES provides transport services 24-hours a day, 7 days a week. While every effort is made to arrive at a convenient time, schedules may dictate arrival at any hour day or night. You will be provided with the estimated pickup and delivery dates. It is important to note that these dates are estimates only and situations beyond our control can and do create delays and/or advances. If delays or advances are significant, affected clients will be called and updated with the revised estimated date of arrival. 12) I Understand that I am responsible to make all necessary arrangements for 24 hour access to the pet(s) I have requested be transported by LNT Pet Transport Services. I understand that, IF NO PERSON IS PRESENT TO SEND pets scheduled for transport and/or LNT Pet Transport Services is unable to contact the responsible pick-up party, we may have to continue the transport without your pet. You will be charged for the full transport cost. I also understand that, IF NO ONE IS AVAILABLE TO RECEIVE the animal and/or LNT Pet Transport Services is unable to contact the responsible drop-off party, there will be a daily $100.00 fee, per pet, to defray the cost of care and the pet(s) will be rescheduled for delivery on the next available transport. The party signing this contract or who scheduled the transport will be responsible for all money due which will be required prior to redelivery. 13) Rescheduling: Cancellations must be made no later than 48 hours before the scheduled pick-up week. Cancellations made after payment has been processed will receive a refund of the total transport price (not including any discounts if applicable) less 25%. Should you cancel after the 48 hour deadline, no refund will be made.
If your animal becomes aggressive during transport, it may become necessary to leave them in their carriers for part or even all of the transport. You will be notified and special arrangements may be required to expidite the delivery of your pet. Should this arise, additional expenses incurred for the expidited delivery of your pet will be billed to you at a rate of $2.00 per mile from the point we are detoured from our normal route to delivery location. 15) LNT Pet Transport Services reserves the right to refuse transport of any pet should there be any misrepresentation at the time of scheduling as to the pets ability to walk or stand on its own accord, or on a leash, and is of a size and weight that 1 person is able to pick up and carry. BY SIGNING BELOW AND BY USING OUR SERVICES YOU AGREE TO BE BOUND BY ALL THE TERMS AND CONDITIONS IN THIS AGREEMENT. Please print clearly
